Summary of this article：In this multicenter retrospective study of 3,591 COVID-19 patients in Japan, early remdesivir use showed no significant mortality reduction overall but suggested benefit in mild respiratory failure. Subgroup analyses indicated improved survival in patients with SpO₂ ≥94%, FiO₂ <0.5, PFR ≥300, and eGFR ≥60 mL/min/1.73 m². Remdesivir may be most effective in early-stage cases without severe respiratory compromise.
