- Summary of this article：This scoping review explores the critical issue of unpaid medical expenses incurred by foreign patients in Japan, a challenge exacerbated by the increasing number of international visitors and residents. By analyzing 17 relevant publications, the study finds that medical institutions, particularly emergency departments, frequently face financial risks due to linguistic barriers, the lack of private travel insurance among tourists, and the exclusion of short-term or undocumented residents from Japan’s public health insurance system. Although some facilities implement proactive measures like prepayment and cost estimation, these strategies show limited effectiveness, and the research underscores a significant lack of robust public policies or legislative support to assist hospitals with outstanding debts. Consequently, the authors call for the development of comprehensive national strategies and further evidence-based research to address these systemic vulnerabilities, ensuring the financial sustainability of the Japanese healthcare system while maintaining equitable access to care for all patients regardless of nationality.
